Yorick talks to the Maiden of the Mist frequently, but do we have any information on her besides yorick's quotes and his upcoming long bio? Is she the ruined queen, or perhaps one of the soldiers kalista was with when it all went down? Also, did the mist corrupt evelynn, or any other champions like karthus or nautilus?
The Maiden is sort of an embodiment of the Mist, being a conglomeration of souls and twisted life energies. It's not a singular entity, it's like the voice of the mist itself given form.

The Maiden is the hodgepodge of all the souls that Yorick has in his cape, not a singular entity. The Black Mist is the massive hodgepodge of souls and dark magic that covers the Isles. Karthus was corrupted by the mist, but he willingly entered the mist to be corrupted, and he honestly got exactly what he wanted: An undead, and therefore immortal, body. It's unknown how Nautilus became what he is now. Nothing solid is know about Eve, but she is associated with the Isles somehow.
Other champs corrupted by the mist: Hecarim - Fused with his horse and now herald of the Isles Kalista - Reborn as a spirit of vengeance Thresh - Given soul stealing powers, and he's perfectly fine with it. Many other champs are associated with the Isles, but were not born from the Black Mist.
I really think that its his old wife/lover due to the quotes and his death animation.
The Maiden is the Black Mist itself given a form. Not all of it, but a chunk of it.
