New Champion Concept - Nero, Eternity's Warrior

I've been playing league for a few years, and have always loved the process that comes with a new champ being released. There's the teasers, the art, the interactions with champs, loading screens, everything that comes out is pretty cool. I've been tinkering with a champion design, and wanted to hear your thoughts on it, whether or not it's good, and what I would to alter to make it more balanced. Keep in mind I haven't thought of names for his abilities, not out of laziness but more because I just can't think of a names that don't seem to rip off other champs, and I don't want this champ to be ripping abilities from other champions. I may not be on any team or department, but still, I enjoy creating this kind of stuff, and want to get better at it. Right, lets get into it.
The Champions name is Nero, Eternity's Warrior. (I'll get into lore after his abilities and play-style)
Passive - Timeless
Nero can only get up to 15% CDR from items, 20% if you count the mastery that gives you an extra 5%. However, Nero gets increasingly more CDR based on missing health, with a cap of 60%, or 65% with masteries. Nero also can cast each of his abilities (save his ult) a second time after each cast for an additional effect, but doing so adds 10 seconds (scales down to 7 with each point in ultimate) to each of his abilities cooldown timers, even if they haven't been used. (After he double casts an ability, all his others except his ultimate are put on cooldown of said value. If already on cooldown, they get more time equal to the time above. If they arent on cooldown, they go on cooldown for the time listed above). Nero can wait 2 seconds to cast the ability a second time before it goes on cooldown.
Q - Dawn/Dusk Dawn - Nero swings his sword in an arc in front of him, dealing 60/70/80/90/100 + ( 90% of attack damage) damage to all enemies a short distance ahead of him. .5 sec cast time, cooldown 20 sec, loses 1 sec with each point put into skill. 2nd cast, Dusk - Nero swings again in a 360 slash, dealing 75/85/95/105/115 + (125% of attack damage) damage to all enemies around him, with double the range (still fairly short range, becomes about a Darius sized Q). 1 sec cast time, adds passive cooldown
W - (Need help with name) First cast - Nero briefly charges then lunges in a straight line (Length of about a Lucian Q, maybe a bit more), dealing 50/55/70/90/115 + (50% of Bonus attack damage) damage to enemies hit. If he hits an enemy champion, he stops at them, but the enemy is airborn for .5 sec and slowed by 50% for 1/1.2/1.4/1.6/1.8 seconds. 15 second cooldown, loses 1 sec with each point put in. 2nd Cast - Nero reactivates to vault backwards to his original lunge starting point. Adds passive cooldown.
E - (Needs Name) 1st cast - Nero leaps to the target location, dealing 50/60/70/80/90 +(80% of bonus attack damage) damage to targets around him, and stuns them for 1/1.1/1.2/1.3/1.5 seconds. 30 second cooldown, loses 1 second with every point put into skill. 2nd cast - Nero Targets and opponent and kicks them a short distance away from him, dealing the above value again to the single target. Adds passive cooldown.
R - Eternal Prowess For 2/2.5/3 seconds Nero focuses his all his power on himself, becoming immune to cc and breaking free from any cc (except silences, stuns and poly-morphs). If an ultimate ability is cast on Nero during this time, he takes 25/20/15% of the damage it would do, and is slowed by 20/15/10%. He cannot be cc'd during his ult time. If the ability was a skill shot, he can reactivate his ult (has another 3 seconds AFTER being struck by ability to do so) and send it back in any direction, having it deal 75% of it's original damage (deals type of damage ability had), plus 100/175/250 +(50 of bonus attack damage) to what it strikes. The skill shot takes on whatever traits it would have had (pass through champs, stops at one champ, radial explosion upon landing), but doesn't carry any effects gained through items (Rylais, Ludens, grievous wounds) or CC. If the ultimate was a point and click, or Karthus ult, He then redirects it back towards the caster, dealing the ratios above. Ults that carry CC (Malphite, Yasuo, Morgana, etc) instead return the damage to the attacker (above ratios), but do not return the CC, but still apply damage based on above ratios. Ults that apply a debuff (Zed) or do damage over time (Miss fortune, Gangplank) or Illaoi's initail slam simply do not damage him for as long as his ult persists. (Zed ult still gives Zed the TP to Nero's location and gains a shadow, but the death mark will do no damage if it activates before his ult duration ends. If it activates after, it will function normally). Nero's ultimate activates on only one ultimate counter, so if multiple abilities hit him, he will block the first one to hit his shield.
LANE Nero is meant to be in the top lane or (stretch here) mid lane. He can be build as a fighter or as a tank, (bruiser?) and excels at engaging fights due to his gap closers and cc. In lane, his standard combo would be W in, cast a q, auto and then reactivate the W to get out. The brief slow and knock up allows you to deal damage then back off, with the slow stopping them from moving to your return point.
BUILD Building black cleaver first is a smart move, as it will give you the maximum amount of CDR from items you can get, as well as health and damage. Due to your passive giving CDR for low health, building mainly tank items, with offensive health items is key. That way, at 50% health, you have up to 50% CDR and still can maintain tankiness. You are gonna be one of the primary engagers for your team, so you want to be healthy and able to soak up damage.
WEAKNESSES Nero is weak to champs that are auto attack based, or have self buff ults, as his ult against them is useless. Nero's ult won't affect an Olaf, Tryndamere, or a Yi, as they are self buffs for ults. Fiora's ult would also be potent, as it would outlast his, and she would still get the healing from hitting 4 vitals regardless of his damage. Early game, using his passive is dangerous, as any champ that has high burst potential can shut him down, quickly. Champions that can also their ults more than once (Ahri, Irelia) pose issues to him, as they once again null his ult. You can counter fast ults like Garen or Darius, but it requires fast reflexes. In lane, he has no sustaining abilities, so playing safe until you are in a position of power is essential.
LORE (Short one) Nero is but one of the many names that the incarnation of Eternity has taken. Once a mortal being that tired of his reality, he studied and sought knowledge on how to become something more. Eventually, he became an undying, immortal being, and took the name of Eternity as his own. Traveling between many worlds, he amuses himself by immersing itself into worlds, and learns the abilities and magics that belong to each universe, constantly expanding his skill and knowledge. When he shifted his eyes to Runterra, he saw aspects of death, powerful humans and creatures that defied reason, and cosmic beings that where sealed to and attracted that world. With a amused smile, He took up the name Nero, and descended to that world to see what attracted so much power.
