Memory and punishment
Sometimes I don't remember why I was punished. (This includes League of Legends bans)
Should I consult a doctor?
Am I forgetting offenses?
Was the punishment ineffective if I forgot the offense?
Am I imagining punishments?

You made an almost identical thread with an almost identical survey earlier today. Please either tell us the point you're trying to make, or stop creating the same thread.

Nope. Largely due to the fact that I am typically an asshole. If I get banned, chances are it is for that reason.
If you're not remembering your punishments and you are finding that this is causing you difficulties, Riot does provide you with example games. I'd recommend keeping copies of these in a pdf file/folder for reference later. If you no longer have it or you don't understand from the games provided why you were banned, I'd recommend sending an email to support for clarification. Then save that correspondence to the file as well. If necessary, also post your games in forums, get feedback on why you were banned so that you understand better, and bookmark/save that thread for revisiting if you find your memory fading.
Not sure most people need to do all this, but hey, everyone has unique challenges and you should do what needs to be done for you. If you find that this is causing difficulties for you in other areas of life, yes, I'd recommend a doctor to see what's up. If not, then a folder for games should suffice.
As for your poll (again) I voted "no," as I've never been punished. I enjoy my green ribbon on my portrait. If I got punished, it would be very memorable for me, as I am a pretty well-behaved person. I've gotten one traffic ticket, and I remember it well.
