The enemy team had such a toxic Irellia, and looking at there past match history, seemed like they had fed a lot. But it's just so nice to see this message appear after reporting who deserved it.

Someone told me to hang myself today, and they didn't get banned I hate how this system only works some of the time
I had a game where our bot laner got offensive, verbally abusive. Flaming everyone and calling people f@$$@T (minus the filtering). He acted like he wasnt afraid, going on about how he "called me a %%% because I was one" and how "Nothing would happen' when even the enemy team reported him and told him he was toxic. "I am not gonna get banned or anything"
We were all still in the lobby when I got the Instant Feedback and the player immediately went offline. :P we all knew he had been hit, as that was my first game of the day. Hmm, nothing will happen eh?
The fact that I've never gotten one of these tilts me so much
