Ribbon Colors and meaning?

I can't find any definitive answers as to what each ribbon means. I get them every now and again but have never known what they mean. I've had a green one, a red one and just now got a yellow one.

Anyone know for certain what each ribbon means?

You can mouse over them to check. Pretty sure green is 'Great Teammate'.
