Don't get me wrong, the system is great at banning people for kys and the N word (feels dumb typing that), but unless you go on a racist rant or afk at fountain the entire game, I almost never see anyone get punished. my last game had a fiddle that build 6 dark seals and was afk most of the game and he sure didn't get punished. I've been sitting here watching the client just to see if he does, silly me. Can we tighten the screws on this system? Hell, I know several people that consistently ragequit or run it down mid once someone on their team feeds and none of them have even gotten a warning.

That is exactly the problem, probably the single greatest problem, with the report system, and there doesn't seem to be any appetite for changing it. Either my reports are all faulty, or damn clear cut evidence of awful chat (in my last game, someone in postgame said the words "u all better give me honor or i'll come rape you" or something very similar to that effect) goes nowhere.

From what I am told only the very last person to file a report gets the indicator, which I find odd since it leads to all these complaints about the banning system from people who want to know the person who wronged them was punished.

Are you aware of the fact that the client does not notify you if the player is punished after they play another game? Things such as AFK or int feeding are sometimes more difficult to detect so they take longer to process and the player could easily play additional games after the game with you before being detected and receiving a punishment.
