I am trying to ask a moderator or anyone knowledgeable to review the chat log of the game that triggered a perma-ban, I don't know where to go to trigger this. I even have photo proof of my enemy team giving me +4 honorable opponent from a fully toxic team. I would appreciate the support from anyone. Thank you.

Won't matter. Their "player behavior" group is a complete joke. I got banned on another account for asking for reports. And then on this account, I got banned for other players requesting to report me, when I was hardly "toxic". This "system" is a complete joke. Can't wait until they end up banning a majority of their player base.
You should have received a copy of your chat logs via email. Check your spam box if you don't see the email. I would recommend you post your logs here. People are going to auto downvoted you unless you post your logs because most people who don't post their logs are trying to hide something. We can't do anything about your ban but if you post your logs you might start getting up votes which might increase the chance of a moderator response.
I'm confused on why my question on "how to appeal a ban" has -5 votes? is this a natural example of a toxic community? I'm just asking for a fair trial I am confused on why so many people down-vote someone who is trying to find answers...
Well, I did go through the whole process, I just skipped out on some information.
Here is the log for the game the system banned me for. Background info, I got a +4 honorable opponent because my enemy team was trolling in all chat aswell, and they were saying that they understood my situation and felt bad for me. Here is the log. At the end of the game, our Oriana said ," I have the right to be toxic, you are doing bad." I muted them for about... 20 minutes and unmuted them at the end after I Congratulated Jinx on killing someone with her ulti.
