It doesn't lead to bans because, unless there is video evidence of someone intentionally feeding (like, say, in Shaclone's VODS), reporting someone for intentionally feeding just doesn't work.

You are aware that there is video evidence of EVERY GAME OF LEAGUE EVER PLAYED, right sunshine?

Do you actually believe that Riot doesn't keep the replayable game-logs? Those are not huge files either. Use the download function of the new client sometimes, a 40min game is barely 5MB.

Well, you can get banned for intentionally feeding, its just that it has to be done often enough and in a very clear way. It does in fact lead to a huge flaw when it comes to banning people, as you will much more likely be punished for chat related offenses (something as minor as calling an 0/20 feeder "bad"), than you would be for intentionally feeding.
