Are there more players like Tyler1?
I meet players from time to time who tell me they're on their 5th and 6th smurfs. Are there are other players who get banned indefinitely?

Plenty of nameless 12 year olds make a new account every couple weeks, not just in league. nothing new.
I believe there's only one other indefinitely banned at present. As I recall, their name is XJ9, and they did some pretty terrible things that likely should have gotten them in trouble with law enforcement, not just Riot.
Indefinite bannings are hard to enforce, and they tend to only go to big names that can influence a hoard of moronic kids into behaving badly, or to people who flat out broke laws.
Despite what it may seem like its their way of stopping them from streaming league not playing so they can't do it to random people nobody knows because they can't enforce it

- Riot has ways of tracking peoples smurf accounts.
- Because of #1 lower leveled accounts are judged much more harshly than higher leveled accounts are to dissuade people from playing on smurf accounts to try to avoid their punishments.
- If they keep on with the same behavior that got their previous account(s) banned then they'll just keep getting banned again and again until they either shape up or ship out.
