Help please, logged on for the first time in months and it says I'm perma banned.

Just logged on because I wanted to try league again for the first time in a while, but then i got a notification for perma ban.

More than anything I wanna know why. I don't know my account email so I can't find the email.

I stopped playing at school because the internet was unstable and I stopped playing because I was bored of league, but now after not playing since 6/11 I'm now banned. I didn't wanna get banned is the whole reason I stopped playing as well. I tried to use ARAMs to clear the bad leave history where my school internet would literally kick me out and I had no control over what happened because it would be fine then just stop.

Can a mod just tell me what happened?

Don't start playing again, just stay away. The game is in an absolutely horrible state atm.

What does the reform card say?

If there isn't a reform card, I've seen them lock people's accounts to protect them from unauthorized log-ins. Hopefully that's why, but as Time said you can open a ticket and find out for sure.
