I'm curious about what the punishment is for someone who leaves 5 minutes in and hands a free win to the 5 people on the enemy team and a free loss for the 4 people that were on his team? This person despite getting reported by multiple people went on to continue playing ranked games as if no punishment was served. If it needs to be mentioned, this person did not ever return to the match.
What gives?
You want them banned over a single game? What if their power went out? What if a real-life emergency forced them to leave the game? What if Comcast/Spectrum/AT&T/Verizon did Comcast/Spectrum/AT&T/Verizon things?
You get a Leaver Buster penalty, whether you're reported or not, if you are absent from the match for too long.

Unless someone who has a spree of AFKing suddenly loses their internet at a time when they're one game from tipping over to a punishment/harsher punishment, they won't get punished.
To get punished for AFKing, one has to have a spree of it. For example; if you're leaving one game a month, you won't get punished. But if you're leaving 5 games a month, you will.*
The system doesn't keep a list of every time you DC/AFK to punish you at a certain number, otherwise everyone would get punished by LeaverBuster at some point. It only keeps track to see how often you're doing it.
*Note that this is, again, an example. I used a random number.

I've suffered through the odd ISP reset or power outage during League and I would hate to get banned for the accidental AFK... :< And I have not lost every game I've had an AFK in, just gotta stay focused on winning!
