Instant Feedback aggregates in-game chat logs only; however, pre- and post-game logs can be reviewed manually; this is where you'd leave a comment in the additional info in the reporting window.

pre and post game lobbies are visible for review, according to a Rioter that responded to a report I sent in through a player ticket. If I remember right, he said they do punish players for actions committed during the lobbies as well, it's not all just in-game

Yes rioters have responded to this question and stated yes they are recorded but not put in the chat logs they give players.
