I've played thousands of games over the years, but this one was in the 0.00001% of most toxic games. It was unbelievable, if someone could please take a quick look I would appreciate it.

I would, but you really need the entire chat log to get a picture of what happened, so unless Riot makes a "what not to do" thread I don't think I can help you.

This board will most likely just get you advice from Arbiters and other non-Rioters, but submitting a ticket will get it looked into by a Player Behavior Specialist. Share with them the match history URL for that match.
