5-10 Minute Social Attitude Survey for LoL players - looking into teamwork & behavior

I helped develop a survey that takes a look at social attitudes of summoners. It will be used for a research-based academic paper for a course that I'm enrolled in. I would greatly appreciate anyone taking 5-10 minutes to complete it, and I believe it will have some interesting implications for teamwork & behavior.

Let me start off, I'm a teacher. I've taken courses in assessment, which you might benefit from if you are going to be doing things that require surveys or questionnaires. I'm not trying to stay your survey is bad either. I'm offering the suggestion that if you get the chance, take a course in assessment. It's worth it on a lot of levels.

My assessment of your survey. (Take it for what it's worth- which isn't a whole lot since I don't know all the particulars of your assignment)

The Values chart (pg 1/5) seems like it will generate a lot of confusion as the scenario your statements are referring to which might bias your results. For instance, we are receiving this from a gaming site so do these statements directly relate to gaming, real life, or generalized themes? Some additional instruction might help give users the incite they need to answer your questions with minimal confusion. You seem to be looking a teamwork as a whole, but from your post here I would have expected it to relate to team gaming or e-sports specifically.

Word choice is key to helping your target audience determine question meaning. Aroused for example... many meanings.. that will cause people to turn away from agreeing more than likely because of the perception of this word in relation to all things sexual.

Formatting issues... on Values Chart pg 2/5 you switch sides for agree and disagree. This could lead to trouble navigating your survey or mistakes creating inflated or deflated results depending on how closely your audience is reading your survey. Going back and forth causes your questions to change line location. This is also confusing for your users. Attention to detail when making something like this can make or break your results and participation.

Watch for redundancy in your questions too. Also, not everyone plays ranked LOL so you need to provide an option for those players taking this survey.

"when others perform better than you do you feel tense or aroused?" A bit weird of a way to phrase that question
