Why didn't I receive a chest/ key?

Don't you get one each game you perform a S, S- or S+ ?

Have you already gotten a chest with Kat previously? Or have you maxed your chests?

Nope I got two yesterday with Irelia and Morganoob I think.

Maxed my chests? What do you mean?

If you go to you summoner profile page, there are three rows of icons to the right of your summoner name. The row on the top has 4 chests the are either gold or grey. If any are grey you can earn that many more chests currently. To the left of the icons it will tell you when you can earn your next chest, which is a 7 day cooldown. This is above the 2 rerolls for ARAM and the FWOTD indicator.

Additionally you can only earn a chest once per champion for each season. If either of these criteria are not met you won't earn a chest regardless of your grade.

Keys are separate from grades and only require wins to earn, however the chance to earn a key appears to be exceedingly low.

If you get a chest with that champ it lowers if not cancels your chances of getting chests. Also if you already maxed out chests for the month meaning you got the 4 free chests + a chest in every champ. Its likely you wont get one, key fragments drop rarely for some reason.
