Whats going on?

I keep encountering errors and being removed from champ select with like 20 seconds on the clock, and then punished for failing to select a champ. I either can't get into ques (click play chose mode have it tell me it failed) or i get in and in like 20-30 seconds get booted out and punished. I get small lag spikes in game but nothing like this. I don't even miss bans and have been removed mid bans.

Right now (the second time this morning) im suffering a 15 min que ban because of having time left being removed from champ select.
