The "All mid" call.

Really, I think the call to group is good more often than not. It feels like it costs games though, because a team that actually has to call "group" is often either already behind, or has recently thrown their advantage (and thus lost momentum).
More often than not, the first team to group up and look for a 5v1 or 5v2 or 5v3 at an objective is successful in that play. If the enemy team reacts to it well enough they may come out further ahead, and if the enemy team was already sufficiently ahead they may be fine after the lost play. In general though, the team is ahead and playing their normal lanes when the behind team makes the call to group and force something - they're successful (in lower elos they're successful repeatedly as the winning team doesn't react after the first time) and start catching up. Eventually the winning team calls to group mid (realizing they're throwing and need to group before it's too late) but at this point the enemy team has a lot of confidence and momentum and it can go either way. The call to group wasn't wrong - it was late.
The word (phrase) I hate the most is when someone claims they're already playing defensively / passively after they've just died (or saying there was nothing they could do). I'm never the one accusing, but I know as soon as I see this that the player is clueless. If you die in lane 1v1, you fucked up. If you can't see that, you're just going to keep doing it over and over again. It's a red-flag for me to get my ass to that lane ASAP and bail them out (which works out most times, thankfully) but they do indeed keep overextending and feeding their lane opponent. Basically need to babysit those guys for the rest of the game (or if they have a late-scaling champion then thankfully they eventually DO get strong enough that you can leave them alone) in order to not lose immediately. Not sure why it's so hard for the 0/5 Yasuo to say "you know what, that 5/0 Riven that was kicking my ass at 0/0 and 1/0 is probably going to kick it again if I go for this CS".
"let me split." when they don't know how to splitpush and they just suicide 5 times and lose you the game
