Has something like this happen to me Saturday. I was playing Sion, somewhat tilting and we were losing pretty badly. So I poorly ulted in river (damn mouse cursor) and weaved too far the the left of where I wanted to go. I swerved to avoid hitting a wall (because I didn't want to) and ended up hitting the entire enemy team under their mid tower. Followed up by a Yasuo ult.
Apparently they were retreating from mid into their tower. So if I didn't mess up I would have missed.

I played Renekton, a Taliyah jumped me and out of panic I mashed all my buttons (including R) and killed her and she said "WHAT THE FUCK WAS THAT?!?" to which I replied (I don't know, I just mashed buttons"
One time I was playing 
and I believe it was a 3v3 at bot. Anyway, I start to get low on health and E into a bush to hide. I kill a low health 
who was trying to hide, with the secondary on my E. It was an amazing mistake.
When you're about to hook the tank, but he moves at the last second and you hook the ADC into a perfectly won teamfight

I did this in ranked with Sona. My ult was on CD with 3s left, I hit a heal in a fight, ran away towards a wounded Zed, away from my Jinx. She died, killing 2 enemies, then asked me why I abandoned her. Then 2 enemies jump Zed, I R, kill one, he kills the other, then we push mid into their base. Jinx was like "Great job!" And I'm like "Totes planned that"
I was just trying not to die.
This is me as Vel'koz. I'll randomly toss a Q out and it kills someone. When I was sure it would miss.
