Well... it's Q E for starters? You hook, pull to, then flay. If it didn't knock back or do damage you may have missed it, it can happen and if your opponent is solid or you don't have the mechanics down well they can escape the combo. I'd suggest try testing it on bots in a custom game to try to recreate the bug and if you can then provide some video and screenshots to submit in a ticket or a bug post.
