When coming out of Rengar's ult with an empowered E, if you spam/use his abilities in a certain way (I haven't figured it out exactly yet), by the time the second empowered ability is available the empowered E actually has a cooldown rather than being reset like it's suppose to be and usually is. I've had this happen many many times (over time, I don't find it as a very frequent bug) but I've never seen anyone else talk about this. Hope you guys can look into this and see what's up. I could be wrong, but I'll keep an eye on it and if it happens again and I notice something I'll write an edit on this post.
