How did I not get an S?

How did I not get an S?
Any number of reasons.
Riot is keeping the exact formula a secret, which means they don't need a neat and tidy tooltip to describe their formulas, which opens them up to run a lot of really involved and complex formulas to determine overall rank. It's a lot more involved than KDA.
They have stated that you're performance if compared to other players in the same region in the same lane with the same champion, so if you run support Annie you'll be compared against other support Annie's.
Riot has also stated one needs to perform in the top 10% of players to get an S ranking, or in the support Annie case: you'd need to be in the top 10% of support Annie's.
Also, general pattern I've noticed is that ranking takes into account how well your lane opponent did, and it also places a lot of emphasis on your CS score.
If you go 5/1/3 with 50 CS vs. a lane opponent that went 2/3/7 with 200 CS: you aren't getting an S. If you go 5/1/3 with 200 CS vs. a lane opponent that went 0/9/3 with 40 CS: you probably got an S.
I know that when i play mid and i roam i get way more S's if i get kills or towers i dont know how they caluclate it but i know that helps
