I'm confused by this because if I'm at full health then why is it that when I cast a spell the passive goes off anyways even though I'm at full health and can't gain anymore health.
I mean maybe this is intentional but I don't really understand how it makes sense for it to go off when I can't even gain more health.

Please, no, never have this happen. That would allow an Ahri that is ahead enough to get full hp now get free mana for harassing to, which is useless to Ahri if she is behind and makes her able to endlessly harass if she's ahead.
