Better Potions.

Current potions aren't bad in early game (they were a lot better in previous seasons). However they become usseles in mid-late becuse at this point it's supposed that champions have items to heal themselves. However NOT all champs have items in their builds to do that, and at the same time with same effectiveness. For example, Tanks have item 3083, ADCs and Assassins have item 3153, item 3072 or item 3812. And with some Mages, you can buy item 3146. In the other hand you have the rest of the mages, supports and some bruisers that have nothing to heal in mid-late.

My idea is very simple, upgrade the item 2003 and even the item 2031 and item 2033/item 2032, to allow them to survive without needing to back to base every time they lost a trade versus some of the other champions. So in numbers, my idea will be (some examples):

For item 2003:

  • Change the flat amount to 160-400 (20 hp / lvl) health.
  • Increase the regeneration time to 20 seconds.
  • Add a new effect: Regenerates the 10% of your missing/maximun health. That amount of health will be regenerated over time, as occurs with item 2012.

For item 2031:

  • Change the flat amount to 130-300 (10 hp / lvl) health.
  • Increase the regenration time to 15 seconds.
  • Add a new effect: Regenerates the 10% of your missing/maximun health.

For item 2033:

  • Change the flat amount to 130-300 (10 hp / lvl) health.
  • Keep mana amount in 75.
  • Increase the regenration time to 20 seconds.
  • Add a new effect: Regenerates the 10% of your missing/maximun health and 5% for your missing/maximun mana (optional).

For item 2032:

  • Change the flat amount to 65-150 health.
  • Keep mana amount in 35.
  • Increase the regenration time to 10 seconds.
  • Add a new effect: Regenerates the 10% of your missing/maximun health and 5% for your missing/maximun mana (optional).

I like increasing the time of regeneration, but for the new effect I'd suggest 5%, regardless if it's missing or maximum health. It'd give tanks noticeably more sustain than other classes, and they should have better natural sustain than other classes without relying on a potion, but this effect could make them difficult to punish; when the squishiest champions are often the ones in need for a pick-me-up early on.
