How do you get Kha'zix 4th evolution?

I have played many games with kha zix and I still didn't evolve 4 skills in one game. I know it's possible I watched NightBlue3 and he did it but how?

  2. both reach level 16.
  3. kill him.

You can only get a 4th evolution when Rengar is in the game.
