Just gave your op.gg a quick look, and can offer a few tips right off the bat.

1: you're playing way to many Champions. Pick 1 role with a main and 2 backups. In your secondary role have 1-2 champs. The goal here is that you learn your champ fairly quickly, and then once you're playing something you're comfortable on you can pay attention to the game itself. Same with role selection, jumping around prevents you from consistently focusing on what else is going on.

2: cut down your deaths. You push to the 9 death zone a lot. I'd have to watch replays to tell you exactly why, but that's way too much. Start with a more consistent goal, keep your average deaths on your main under 4 or 5. If you get that down solidly maybe drop it to 3, but that's a ways off. Every death is not only 300+ gold to the enemy, but now the game is 4v5 for some period of time.

3: ward. You buy like, 1 control ward a game. Sometimes 0. Your trinket appears to frequently be sitting at 2 charges. Vision is broken op, there's a reason they keep nerfimg it for high level play. Ward more. (and watch your minimap)
