I believe it's on mana expenditure, so as long as you're spending mana while it procs, you should be good

His w cost mana every second, so it'll proc it. Pretty sure things like jayce's r won't proc it.

Every mana expense, including toggled abilities, will proc a Tear. Though abilities with no mana cost or abilities that cost Energy are not affected.
