Need help understanding "Collateral Damage"

I have never understood the tooltip for Graves' ultimate.

It seems there are 2 damage sources: one for direct impact, and one for aoe behind the impact.

So my question is can a champion if running away from Graves be hit by both the impact and aoe? Or is that champion immune to the aoe damage if hit by the impact damage?

I would really appreciate some help because I don't understand how to use it effectively. It seems to do so little damage to me until late in the game.

You'll only get hit by one or the other. It's just a Direct hit = more damage type spell.

You know how there are some AoE spells, like Zigg's ultimate, where it deals more damage if you hit them directly?

That's basically what this is, except it's a skill shot.

You should only be hit by it once. The only spell that I know of that you can get "hit" twice by is Varus' Q if you flash/blink into it again.
