I want to improve my gameplay but i dont know how.

Hi guys, I just want to ask some tips of what could i do in a match to improve my skills like a training when i focus to a 1 or 2 thing .Now i want to play ryze better , I know , is a hard champ but in some matchups i win my lane .... for an example i can got 5/0 but then my score go worse and worse and worse . I die like a retarded everytime and i dont know how to stop that .In early game i dominate but in mid and late game i play like a noob.In tf i dont know what i do and again , die like a retarded . I think you got this guys , in early i play good but in mid and late i dont know what to do ... thats why im asking you what to do to improve my gameplay not just for ryze ... but at all gameplay.I appreciate all the help . What do you have at your disposal? I have ult, I have zhonyas, I have flash, I have ghost. Great. I am also ahead and can 1 v 1 or maybe even 2v1. Great. Go push a side lane and get solo Exp and gold while paying attention to the map. Whoever is not on the map you should assume is coming for you. Can you Deal with them? Think in advance I will need to dodge the lux bind. Think about your combo to root people in advance not just reactively. It's really just that... thinking in advance what they will try to do WHEN they come for you cause that's what you want, to draw pressure and free up other turrets/objectives. But you also want to make sure you're losing only cooldowns and not giving up kills. when you only have a few cooldowns up but still fed group with your team and force fights around objectives, when you have nothing up, No flash no ghost no zhonyas no ult, play safe and farm your own jungle and stay near mid (or wherever feels safe) till they are back up.
