Hextech Crafting Rewards?

I've earned an S rating twice where I did not get a chest. I did get a chest for another S- rating though. Is this a common issue, or is there some part of the system I'm missing? I really want that skin...

You can only get a hextech chest once per champion that you own per season. And you can only get 4 chests per month from getting the S ranks.
