It just sucks, and 48.7% win rate of it just explains how undertuned it is compared to more popular spells like ignite. Its that bad, that you are forced to dodge que if your support goes exhaust, cus it really provides 0 pressure.
Suggestion: Either buff the damage reduction numbers or give it some special effect like grevious wounds or something, because ignite is just broken.
It honestly is crap on many supports. It's still situationally useful though. Not going to save you from Kindred or Khazix but it will totally cuck a Yi dive. Also support isn't the only one who can take it. I don't know why but y'all ADC players don't seem to realize Heal is equally effective if your support takes it, and if your support is going to be behind you when you go in you'll have more opportunity to drop an offensive summoner spell than they will. If you're going to play Lucian or Tristana, consider taking Exhaust or Ignite yourself and letting the support bring your Heal. Unless of course you have a frontline support like Leona that will 100% be in range to use it when the fight goes down.

Heal reduction is very valuable in a duo lane especially because Heal is guaranteed. Exhaust isn't bad, just more situational than additional damage is.
