Your average bronze player looking for help

So before I start I am a bronze player, I started the day after jhin was added live and I have played almost everyday since, and I will say it I'm hot bronze trash, I suck, I am awful, my team isn't bringing me down I'm tearing them down, and I need help. I hop around roles a lot but mainly I play and I'm fairly competent (ik ik I'm a bronze lee) I have paths, builds, and correct runes I am spotty with how many ganks I have but generally I'm positive at 20 minutes (scores generally like 5/3) and while I know that falls off I should be decent in midgame. I generally die 4-6 times in the time of 25-34 minutes which is crazy high. Around 35-40 I stop dieing so often, and I die around once every 8-10 minutes after that. But I feel I give up everything in mid. And I probably have other issues, but focusing on this any tips to making choices in the mid to late game? (this is meant for overall but if you have tips specific to then I will happily accept them)

If you have issues with my build also please bring them up, generally I go with dark harvest as my keystone going second tree precision
