Having problems going HAM, need help from High elo player

I can build a lead but I notice that I am not aggressive enough and I am still giving my opponents too much room to breathe and comeback. I want to be like that diamond plus smurf (see the twitch in my match hist) who know how to go ham and dominate games. He was a timebomb, i warned my team what he was going to do and they still fed him and at one point he had everyone at the fountain and routinely ends game's 27-3 or something like that.

When I get lead I lose vision and the whole enemy team is usually grouped nearby so if I fight I usually get ganged up on and lose. How do i get around this?

When you get fed you should try to show your presence around the map. Roaming is a good way to do that when you win your lane. You should also maintain a lead however possible and not accidentally throw it. Play aggressive but safely. Do not wander off alone since the enemy team will see this and try to pick you off since you are worth a lot of gold.

Knowing power spikes of champions is the biggest part of it . Learn who stat-checks who, and how supports factor in. Map awareness is another important part of it. See 3 top? Go ham 2v2 botlane if you out-stat them, or know you can outplay a even or slightly-behind stat check.
