What constitutes a "LOSS PREVENTED!" status?

I have just recieved a loss prevention because my entier team DC's contantly for a good portion of a game.

But I had another game that went full CRASH!!!, and It was not a loss prevention, even tho the other team was able to reconnect, and drop 4 turrets before anyone got back in on my team.. (Which was me..) the rest of my team didnt come back for about 5 min while I tried to fend off 3 attackers.

The Loss prevntion game was a normal, the Full Crash was Ranked.. Is that why?

I think it depends if the problem was due to Riot Servers (which is a loss prevented) or if it was some problem with the player's computers (dropped connection, drophacking, etc), then it's not a prevented loss (if it's drophacked, it'll be prevented, or rather, erased from the servers, so it won't count as a win or a loss, it'll just disappear, from what I know at least)

Loss Prevented actually needs to be turned on by Riot, and then it'll apply for any match (ranked, normal, even bots) lost while it's on. Riot usually turns it on when servers start acting up, but never soon enough to actually cover every match that goes sour because of the servers acting up (invariably they'll notice the instability with a delay, and matches that end during that delay won't get Loss Prevented). Once servers stabilize, it's turned back off. It doesn't actually matter if there was any instability during the match or not (not every match during a period of server instability has issues and not every match with issues means the servers are unstable in general).

Sometimes matches crash because the server they're hosted on crashes, but the rest of the servers are fine. That is unfortunately always a possibility, although it should be very rare.

If you get a Loss Prevented, you shouldn't lose any LP (in ranked) and it shouldn't count toward your losses count (in any mode).

I've gotten a couple losses prevented by reporting a feeder before, which was nice.

League of Legends support has a whole article on this. You can find it here. But, looking at the article, it's a little flawed. Teemo players are never allowed to have a loss prevented, especially if playing 3 games in a row.
