It's not possible to create a concrete list of requirements; it's based on your performance compared to all other players using that same champion in that role, so the "requirements" fluctuate as players do better or worse.

I know of one trick to get S rank as support. Since a portion of it is how much you ward, you can just get a sighstone and repeatedly drop wards on your fountain. Since sightstone refills itself at fountain, you can do this indefinitely. Basically anytime you back just dump a ton of wards on the fountain. It won't guarantee the S, but it will satisfy the warding requirements for it at least.
