Are the item number slots the numbers on the keyboard you use to activate the items? Or are they always active once you buy them? So confused...

Item stats and passives are always active after you purchase them. Abyssal Scepter is a good example of this.
Some items have "active" actions that require the appropriate button push to use them. Sightstone and anything called an "Eye of", Blade of the Ruined King, are good examples of these.
in addition to what ramp said, you are able to rebind your item slot keys to make it easier for you to hit them when you need them. personally, i like to bind one of them to "t" for my wards or potions
They are always on, unless they have 'ACTIVE: [words]" or "UNIQUE ACTIVE: [words]" in their tooltip. Items with Actives need to be used, and the number is the keyboard key that is used to activate them. Most items with an active have a border around their icon in-game. You can click and drag to rearrange items. Items with actives are usually better on 1-3, as you need to move your fingers less to get from QWER to activate them in a pinch.
