Its his only damage ability so it doesn't make much sense for it not to apply on hit effects. And he uses his swords to deal damage but he doesn't apply on hit with his Q? If you did he would have better scaling becuase he could proc conquer with his Q and rageblade would work too yasuo Q does damage in in line in front him and it applys on hit
If they did this, it would only do it on the first target hit and it would be lame. Riot doesn't like the idea of AoE abilities proccing onhits in an AoE because they are afraid to do cool stuff. But to be fair it would be really stupid with Tiamat since that item is so easy to rush.

There are two instances of on-hits being applied in an AoE in the game currently;
Gangplank's kegs apply on-hits from the triggering attack to all enemies around the keg. In the case of Tiamat, Cleave will proc around the keg itself.
Shyvana's dragon form Q applies on-hit effects twice to all enemies in a cone in front of her. Tiamat will only trigger Cleave on her initial target, though, because Tiamat has an internal CD of 0.05 seconds against new targets specifically to prevent Shyvana's Q from breaking it.
It did this on release, surprise surprise, it was busted
