Guys pls help me! What determines whether you will become S-, S or S+ ? PLS I NEED A HELP!!! And sry if my english is not good XD

Your percentile performance compared to all other performances on that champion in that role. For example to get an S+, your performance needs to be in the top 2% of all performances on that champ/role combination.
Lots of things go into 'performance', from kda and cs, to warding (both clearing and placing), to objective participation and kill participation, and probably a few other factors too.
Literally everything you do in game determines what you get. Kills, Deaths, Assists, Kill Participation, Wards, Objectives, Towers, Inhibs, CS, Gold.
Maintain a high CS... If you can, try to keep @ 10cs/Min
Meaning @ 15minutes try to have around 130cs, and dont die. and you'll be peachy with an S
