How bad is your name screening?

i just had a game on an alt account that had someone with cancer as part of their name and another with KYS as part of it followed by racist slurs. These where separate words in their names also not hidden in their names. If you can have a computer program look for these words in chat how can you not figure out how to keep these from becoming peoples names. I'm thoroughly convinced a monkey could do riots job better than they are.
