If i craft skins for DJ Sona, Pulsefire Ezreal, or Spirit Guard Udyr, would I get the icon and the banner along with it?

Yes u will .... got SGU from HC and i got the whole icon pack with it ... as you can see from my icon
Yes, the icons are tied to the skin, so you get them when you get the skin. The banners are tied to the icons, so the whole package is just that.
I crafted Pulsefire Ezreal a few weeks ago, and can confirm that you do get the icon/banner
