Played a game last night with it and just...shat on the enemy team, not exactly sure what I did. Now i know it says every 4th attack but does poison damage count as 1 attack? or what if you poison 2 enemys at the same time?

After** four seconds** in combat (basically, taking damage from or dealing damage to anything), your next basic attack will heal you for and deal damage equal to a percentage of your maximum health.
If you leave combat before consuming the buff it will disappear. If you remain in combat after using the buff, you will get it again in four seconds so long as you remain in combat.
