I Cannot Get an S for the Life of Me

So recently I played a game as mid lane , a game I believed was S worthy. Apparently not. The game lasted an hour and ten minutes, I had the most kills in the game 18/7/16, I had 405 cs collected, and I won the game yet I only got an A+. At this point I don't even know what I'm doing wrong. Can anyone point out to me where I messed up?

CS and too many deaths. Yes, 405 is a lot, but it's only 5.7/m in that long game. Need to be 6+ and anything more than 4 deaths really pulls you down.
