I went 11/1/17 on 
a very hard champion to play against a Yasuo and Yi.
I had 48 vision score, 3 turrets destroyed and 1 inhibitor.
208 CS and 15,683 gold
And yet I get an A- as my grade.

Why?
AND I didn't get a chest although Lux got an S.
Control wards purchased: 0 Damage to champions: 25.3k, which is lower than your Top (45.4k), ADC (33.4k), and Support (28.7k). The only person on your team you beat out was your Jungler (18.1k)
I'd bet those two are what did you in.
Correct me if i'm wrong, but...
K/D/A isn't the only thing taken account when scoring happens. Every stat you see in the graphs / overview / stats page contributes to your score. If you did little to no tower damage, your chances of getting at least an S- are lowered. Maybe you managed to last hit 11 kills but didn't do much player damage or didn't have a good CC score. Every bit counts, so maximizing everything you can do efficiently is the best way about it. Hopefully next game you get that S- or higher you deserve for doing well on
