I seem too be struggling with 
quite a bit. I usually win lane quite easily but the moment it hits mid to late game I get bursted down before I can even get my passive off, my healing seems kind of mediocre, I find it difficult to engage without getting kited by your everyday ADC, and overall the champ just doesn't feel right to me.
Does anyone have any tips for me or can relate with these issues?

Darius is the worst top at the moment after the conq nerfs, you'll be okay after next patch maybe gl tho
Don't bother. Lest you do, an entitled platinum Yasuo main who knows not better than to walk up to Darius and start autoattacking him in the face will downvote you because Darius is 'obviously op'.
Honestly speaking, there's no reason to play Darius in this meta when you've got divers such as Renekton and ranged champions such as Urgot who do everything Darius does but better.
All of off meta top lane feels like this.
darius doesn't teamfight, and when he does he usually flanks and cleans up rather than engage for his team. when he's not doing that, he's usually top or bot lane splitpushing and looking for a 1v1 which he is good at.
Darius is a lane bully and it's very easy to win lane with him The problem is he falls later because as you said he gets bursted
Personally I play him BC then full Tank , it works well
Darius is meant to win early and close the game out.
Late game you will get outscaled and wrecked by Marksmen and Fighters as they buy penetration / anti healing items.
Being kitable is the price paid for playing a class that physically cannot lose once they are in the fight. (Unless the enemy is massively fed of course)
There may be something to your point. I personally find Darius super easy to counter with Malphite, he is always my bitch top.
Fellow Darius Player with M7. It's the meta right now, but expect to get downvoted. Only recently I have been told Darius is still S+ Tier and one of the best toplaners right now. :3 Only things I can think off are BC + Steraks, Trinity is worthless for me, since I just get melted and I don't need Damage when I can't even get it off. Take Phase Rush/Aftershock since Conq isn't worth the trade, also Triumph and Last Stand/Tenacity Runes if needed. Ghost is pretty good too against very mobile champions, especially in low Elo where no one uses a TP disadvantage.
Never trinity, it is what makes darius weak. I get black cleaver, frozen heart, spirit visage, pure damage afterwards, get into a fight, get my 5 stacks, get my ult and then I win the game. Build tank, save a darius.
Probably know everything I've typed below, but sometimes someone stating what you already know helps.
Think of Darius as Nasus. He does extremely large amounts of damage, can heal a boatload, but can be a joke when it comes to teamfights as he just gets peeled to all hell.
A lot of Darius' strength comes from the fact that he has a huge advantage during laning phase. 1 kill and 30CS usually means that the opponent achieved their win condition and you lost lane. Try to make the enemy laner a non-factor as you can quickly become one yourself during open teamfights.
Darius is immobile, so see if you can't get deepwards into their jungle somehow. Darius can pull off some quick 1v2's, but is too immobile to get out of a fight you don't want to take.
Other than that, focus on coordinating ambushes and picks with your team.
