Black cleaver is providing just as much health as any tank item
Along with phantom dancer, these two items are providing about the same defensive stats as, say, dead man's plate, just by building pure offense. There's an adjustment idea for you.

It's supposed to. It's why it doesn't come with any resistances attached to it.
Juggernauts and tanky Divers are meant to build Black Cleaver and use the health as a defensive stepping stone so they can survive into the mid game where otherwise they would die and not get a chance to deal their damage. Same reason Titanic Hydra and Sterak's Gage have health attached.
The only champion I can imagine using BC+PD is Graves. And you should just ban him regardless, he doesn't need those items to be oppressive.
