So, during laning phase I think I'm fine, but, afterwards, I can never make the calls on my own, only respond to the calls of my allies. I have very little understanding of what I SHOULD be doing, rather than what I am be doing, does anyone have any tips, or help?

the reason being because it depends on what champs you play and what you WANT to be doing and what you FEEL you should be doing.

nasus is a champ that either wants to just peacefully farm top forever or bully the other guy
Camille wants to fight you.
Both go top but nasus can freeze lanes and survive a 1v2 top while Camille struggles with this.
Earlygame I'd expect you to get a lot of cs as nasus if possible but you also want to get as many Q stacks as you can. This can be done by lowering cooldowns which will typically get done by building defense or triforce or something
Camille on the otherhand just needs to be ready to fight and be ready to gank.
Camille can engage from far away making her great when the jungler comes to gank but when she gets countered its gonna be ugly because she isn't like nasus where she can just bs the damage away with a healing passive.
As Camille you need to either commit to the fight with 
or 
or you need to just be a realist and take 
knowing you may have to go back or die. In anycase if you take everyone knows that you aren't gonna be as strong as you could be and that earlygame is the time to trade with you if they can. It depends on your opponent as well of course.
More than likely on either champ you will need to practice farming minions and you will need to learn your matchups.
What you build early is going to determine your damage or utility for later game and for the gathering of resources during the earlygame.
If you get no defense on nasus for example its gonna be hard to teamfight but you cans till get a lot more farm by getting an offensive item that helps you farm but there's a limit to how much damage youll actually need.
Later in the game you will need to do what you set out to do which is why I'm talking about the early game so much. Your strategy for the game, even as it evolves, is the solution to this. You need to play with intent, with an idea from level 1 what is you wish to do.
As nasus you have certain options like splitpushing or being your team's frontline fighter. To do this you need to understand each objective and what it actually takes to get said objectives or rather how little it takes. Once you understand this it is a lot simpler. But there is no one answer for flexible champs like this unlike an adc that needs to just keep dealing tons of damage

laning phase only ends when one team decides to make a move on their opponent because they feel they have an advantage or they're trying to recover from a disadvantage.
once you're starting to either take or lose turrets thats when you know it's time to start looking for ways to win other than just farming minions.
every role has their own ways to make plays after laning phase. you will have to be more specific if you want better advice.
