How is it balanced that Sivir gives her entire team ghost?

as a corollary, why isn't she picked more often in competitive play, even if solely for this reason?

She doesn't scale as hard as other ADCs such as Draven, Vayne, Tristana, or Kog'Maw. Also, team utility usually isn't what an ADC is picked for. They are usually picked for kill potential.

She used to actually be a rather large factor in the competitive scene up until about early/mid S4. There's even a team comp named after her: "Sivir Stampede"

Sivir's biggest problems, in my opinion, revolves around her mana flow. About the time Sivir fell off was when she received titanic nerfs to her laning phase. Her Boomerang's damage reduction is far higher than before, essentially meaning trying to harass through minion waves became harder to have an impact. Her W cost was increased un-proportionally to her E's mana return, meaning it's so much easier to starve on Sivir if you aren't extremely careful with your skills. Basically, patch 4.5 pushed to try to make Sivir into a "high risk high reward champion", and while the definitely increased Sivir's risks, her rewards for it weren't proportionally worth it.

And then the ADC itemization rework came along and essentially shot every AD Caster type ADC (Sivir falls into this category) in the knee when they reworked the Bloodthirster.

Because you're giving up a potential source of meaningful damage. It's scary in the right situations, though.
