You only participated in about 1/3 of your teams kills, yes the cs is good and you didn't feed but to get an S+ you really need an outstanding score along with it. Add on 5 or more kills to your score and you probably would've gotten an S at least.

team fights aren't something that is considered I believe, over kills are. and it's compared to time, so I guess I agree that 4 kills and 12 assists aren't alot for a 50 min fight

It compares your score to other Teemo player's. You were lacking in Kills, but your CS was over the top so thats why you got an S-.

it compared me to other Teemo Tops* but yes I guess so. the CS you want to stay around the same digit of the timer (200 at 20, 300 at 30, etc) so yea I always thought staying even in kills was good enough, but I guess not
