TSM is damaged at this point and will not be able to recover until next year.
In that time, Dyrus, Turtle and Lust need to go. Santorin needs a kick directly in the ass.
Additionally, and it pains me to say this because I don't think it's his fault, I think Loco needs to be moved to analyst and a new coach needs to be brought in.
Lastly, Regi needs to stay out of the team directive. I like Regi as a person. I think he's a good friend to the players. His approach when dealing with a lot of the players is extremely counter-productive to positive movement. If you watch the TSM legends series at all, there was a bit where Regi and Turtle had a pretty shitty exchange during the Korean Bootcamp, which was Regi being a %%%%. Now, that could be a cherrypicked scene and he's not normally like that, but just based on what I've seen, I really don't think his presence is helpful when the team is struggling.
Now with that said:

It physically hurts me to talk about Meteos coming into TSM, however, I'm hoping that Meteos is having/has had an xSpecial style revelation after leaving Cloud 9. BunnyFuFu coming in to replace lust would be a literal wet dream because it shifts the shotcalling away from Bjerg who doesn't like it anyway. Santorin doesn't necessarily need to go, but, he needs a fire under his ass.
Maple would stick mostly to utility carries. Sivir, Ashe, etc.
