Fiora's q isn't prioritizing vitals for me

I've been playing Fiora a bit and I like her for the most part, except her q seems to never hit the enemy's vital. I've been trying to harass in lane with my q, aiming to hit their vital, but it never does. I am sure that in game it says lunge will prioritize the vitals, but I can't recall hitting a vital with my q in lane in the last four games I played with her.

The targeting is definitely not optimized, it actually reminds me of Gnar's boomerang when he first came out, but it will try to go for vitals. Most of the times if fails in my observations are:

When you are at the very furthest end of it's range. I've actually seen it strike away from a vital to hit an incredibly low minion behind me even though I was definitely in range to hit the champion
When you are incredibly close to your target. If you are too close to the target you almost seem to be in between the character and the vital, so the vital isn't counted as hit or targetable
At the edges of the vital, where it borders other vitals, it often doesn't pop the vital even though you can clearly see the Q effect go through the vital. This is probably just a visual to hitbox miss-match.
