Taking damage, but death recap doesn't tell me from what?

Not sure if this is a bug, but I've been getting it a lot. I keep taking damage from something that isn't listed in the death recap.

It's just a blank square with a damage next to it. Usually it's magic damage, but once I saw it and it was over 1k physical damage. No indicator what it was from. It didn't tell me if it was a basic attack or if it was an ability. Nothing. Just a blank square.

I woulda put a screenshot, but taking one before the death recap disappears is hard to do.

Anyone know why this is happening?
